PIPEDA Supplemental Access Request Sent to Manulife Privacy Office

Rod sends a formal supplemental access request under s. 8 of PIPEDA and Schedule 1, cl. 4.9 (Principle 9 - Individual Access) at 12:48 PM to three Manulife privacy addresses. The request supplements, and does not replace, earlier access requests, and is directed principally to personal information not contained in the June 3, 2026 claim file production, including records created or entered after that production, late entries describing earlier events, and responsive records that predate the production but were omitted. It expressly covers the second appeal that produced the July 8, 2026 reversal, the internal medical consultant review referenced in Manulife's June 18, 2026 communication, occupational information relied upon, and - without any date limitation - the complete Work Facilitation and return-to-work meeting file including Michelle Stewart's notes, the May 5, 2026 preparatory call, and the May 7, 2026 cancellation decision. Delivery succeeded to two addresses; [email protected] bounced with SMTP 550 5.4.1 (recipient address rejected), so the request was validly submitted to the two delivered addresses.
